Many things could be causing that to happen. It could be that the cast does not unroll with enough power to give compltet turnover of the line leader and fly.

If you are using too light of a leader or tippet for the size of fly you are throwing that could do it. For the most part, fluffier flies will cause the most problems. By fluffy I mean a fly that is bulky and air resistant but not heavy, like a bushy dry, a hopper, or a deer hair bass bug. I wouldn't use lighter than 4x for bushy dries or hoppers and not less than 10lb/2x for bass hairbugs.
